The family's been very private about what type of cancer actually killed him - some type of lymphoma or throat cancer. But if he had lymphoma, to me that would explain a lot, given his penchant for working many hours and sleeping little during times where he was putting out a novel. Physical fatigue from lymphoma (or similar blood-related diseases) will crush ability to focus and concentrate.
