I had one way back in my playing days ... painful as heck. Think of it as a ligament unnaturally rubbing/sliding against the bone during movement.

Because it's a ligament as opposed to a major muscle it takes a lot longer for the swelling to subside (altho the pros of course have ready access to cortisone shots & other technique that us mere mortals don't)
